Eligible Department of Veterans’ Affairs Gold Card holders may be supported through the Coordinated Veterans’ Care Program. This program involves collaboration between the patient’s usual GP and other healthcare providers to support ongoing management of chronic conditions, regular review, and coordination of care. Patients aged 75 years and over may be eligible for an annual health assessment conducted by an independent doctor. Approximate duration: 60–90 minutes The assessment allows time to review health concerns, daily functioning, and ongoing care needs Advice, monitoring, or referrals are provided where clinically indicated
