Aged Care Services May Include

Huntlee Healthcare provides general practice services to support the healthcare needs of older adults. Services are available to patients aged 65 years and over, or 50 years and over for Aboriginal and Torres Strait Islander people, in accordance with eligibility guidelines. Care is delivered by independent doctors and focuses on appropriate assessment, monitoring, and coordination of care.

Aged Health Assessments

Aged health assessments are offered to support the review of physical health, mental wellbeing, and functional capacity in older patients. Assessments are based on individual clinical needs and may include review of medical history, current health concerns, medications, and lifestyle factors.

Scope of General Practice Services

As part of general practice care, doctors may also provide or coordinate services related to: Children's health and immunisations Men's and women's health Shared obstetric care General health assessments Travel medicine Services are provided based on clinical assessment and individual patient needs.

Medication Management Reviews (Approx. 60 Minutes)

Medication management reviews may be arranged to assess current medications and identify potential issues related to use, safety, or effectiveness. Reviews are conducted by doctors and may involve coordination with the patient's nominated community pharmacy. Home-based consultations may be arranged where clinically appropriate and where the patient is eligible.

Coordinated Veterans' Care (CVC) Program

Eligible Department of Veterans' Affairs Gold Card holders may be supported through the Coordinated Veterans' Care Program. This program involves collaboration between the patient's usual GP and other healthcare providers to support ongoing management of chronic conditions, regular review, and coordination of care. Approximate duration: 60–90 minutes Participation and eligibility are subject to program criteria

Annual Health Assessment for Patients Aged 75 Years and Over

Patients aged 75 years and over may be eligible for an annual health assessment conducted by an independent doctor. Approximate duration: 60–90 minutes The assessment allows time to review health concerns, daily functioning, and ongoing care needs Advice, monitoring, or referrals are provided where clinically indicated

Preventive Care

Preventive care consultations may include assessment and discussion of relevant screening options based on your age, health history, and individual risk factors. Doctors may provide guidance on screening programs and preventive health services where clinically appropriate.

Healthy Lifestyle Assessments

Healthy lifestyle assessments may include review of relevant health indicators, including lipid levels, glucose levels, and blood pressure, as part of preventive care. Assessments are conducted based on individual clinical need and may support discussion of lifestyle factors with your doctor.

Immunizations for Seniors

Eligible patients may receive government-funded vaccinations for influenza, pneumococcal disease, and shingles. Eligibility is subject to age and program criteria — please speak with our team to confirm your entitlements. Where government-funded vaccines are not available, fees may apply.

Advanced Care Planning

Our doctors can provide guidance on advanced care planning, including discussion of long-term care preferences and short-term care options. This process supports patients in documenting their healthcare wishes. Doctors approach these conversations with compassion and respect for patient choice.

Caring Palliative Support

Our doctors and nurses, in collaboration with specialist teams and support services, provide palliative care support as part of general practice. Care is delivered with a focus on comfort and wellbeing for individuals with serious or life-limiting conditions. Referrals to specialist palliative care services are arranged where required.

Nursing Home Visits

Our independent doctors provide nursing home visits by prior appointment. Medical support is based on individual clinical needs and may include assessment, medication review, and care coordination as appropriate.
