There are two types of Chronic Disease Management Plans available for patients:

GP Management Plan

A structured plan to help patients manage chronic conditions with regular medical support.

Team Care Arrangements

A coordinated approach involving multiple healthcare providers for comprehensive care.

Eligible Patients

Eligible Patients

If you're eligible, our nursing team will help develop your GP Management Plan (GPMP) and Team Care Arrangements (TCA), including referrals to Allied Health services. This process includes a detailed medical assessment and coordination with healthcare providers, taking around 30 minutes. Eligible patients with a valid Medicare card can access this service at no cost.

Ongoing Support for Your Health

Eligible patients can access up to five subsidised Allied Health visits each year, with a new plan available every 12 months. Our GP Care Plans help manage chronic conditions effectively, with independent doctors providing guidance
