Supporting Your Health and Well-Being

BOOK APPOINTMENT

There are two types of Chronic Disease Management Plans available for eligible patients:

GP Management Plan

A structured plan to support patients in managing chronic conditions with regular medical review.

Team Care Arrangements

A coordinated approach involving multiple healthcare providers to support ongoing care for eligible patients.

Eligible Patients

If eligible, our nursing team can assist in developing your GP Management Plan (GPMP) and Team Care Arrangements (TCA), including referrals to Allied Health services where appropriate. This process includes a medical assessment and coordination with healthcare providers, taking approximately 30 minutes. Eligible patients with a valid Medicare card may access this service at no cost, subject to Medicare eligibility criteria.

Ongoing Support for Your Health

Eligible patients may access up to five subsidised Allied Health visits per year through their care plan, subject to Medicare eligibility criteria. Care plans may be reviewed and updated annually. Independent doctors provide ongoing guidance and coordination of care as part of the management plan.
