Chronic Disease Management

Chronic diseases affect millions of Australians, affecting their quality of life. At Huntlee Healthcare, our independent doctors provide care through GP Management Plans (GPMP) and Team Care Arrangements (TCA) to support the management of long-term health conditions.

Chronic Disease Management Plans

There are two types of Chronic Disease Management Plans available for eligible patients:

GP Management PlanA structured plan to support patients in managing chronic conditions with regular medical review.
Team Care ArrangementsA coordinated approach involving multiple healthcare providers to support ongoing care for eligible patients.

Eligible Patients

If eligible, our nursing team can assist in developing your GP Management Plan (GPMP) and Team Care Arrangements (TCA), including referrals to Allied Health services where appropriate. This process includes a medical assessment and coordination with healthcare providers, taking approximately 30 minutes. Eligible patients with a valid Medicare card may access this service at no cost, subject to Medicare eligibility criteria.

Ongoing Support for Your Health

Eligible patients may access up to five subsidised Allied Health visits per year through their care plan, subject to Medicare eligibility criteria. Care plans may be reviewed and updated annually. Independent doctors provide ongoing guidance and coordination of care as part of the management plan.

What is a chronic condition management plan?+

A chronic condition management plan is a structured plan created by your GP. It sets out your health goals and the care you need. It can also help you access certain Medicare-supported allied health services.

How often should I review my chronic condition?+

Review frequency depends on your condition and how stable it is. Many patients have a review every few months. Your doctor will advise a schedule that suits your needs.

Can my GP coordinate care with other health providers?+

Yes, your GP can work with allied health providers and specialists. This helps keep your care consistent and organised. Referrals are arranged when they are clinically appropriate.

Are chronic disease appointments bulk billed at Huntlee?+

Standard and long GP consultations are bulk billed with a valid Medicare card. Longer appointments are often useful for care planning. Reception can help you book the right appointment length.
